2. Primary 4 and 5 Academic Reports

Upload reports for 1st Term, 2nd Term, and Whole Year.

Diplomas or certificates of special skills and achievement awards (optional).
3. Applicant’s Photo

Size: 40mm (width) x 50mm (height)
4. Identification Documents

Upload a copy of the student’s Hong Kong Identity Card, Passport, or Birth Certificate.

For non-permanent residents, upload the visa/permit with an endorsement issued by the Hong Kong Immigration Department, indicating permission to study in Hong Kong.
5. Application Fee Payment

Upload the bank receipt for the HK$75 application fee.
6. Primary Six Student Record Form
-
Email a copy of the form to [email protected] on or before 31 January 2027. Parents will receive this form in December from their child’s primary school.

Note: Students studying in overseas or international schools are not required to submit this form.

Applications that are not completed online will not be processed.
